PRIMARY CARE PHYSICIAN:
At some point in our lives we are all patients. Having a primary care physician to help YOU manage YOUR health care is one step closer to good health and patient satisfaction.

WHY IS IT IMPORTANT?
It’s important to establish a trusting relationship with one physician who will get to know you and assist you in better management of all of your health care needs, including preventative care, as well as any acute or chronic care conditions.

  • Having a primary care physician is important because when you see several different physicians without having one primary care physician manage YOUR care; you may not receive the kind of quality health care you want and deserve.
  • YOU deserve consistent high quality healthcare. Evidence shows that patients who have chosen a Primary Care Provider are more likely to receive proper prevention screening and consistent management of chronic illness.

CARE TEAMS:
As part of our goal to improve patient care, the Health & Wellness Center staff can help you choose a Primary Care Physician and establish yourself with a Care Team, which consists of you, your medical provider, an RN team manager, an LPN or CMA, and a medical scheduler. These will be your “go-to” people, or resources, in a patient-centered care environment and patient-driven care model.

The Care Team approach – with a designated primary care physician of your choice – allows you, your physician and the other staff to develop a trusting, supportive relationship. This provides you and your family with the opportunity to take a more active role in managing your health care.

The Health & Wellness Center encourages
all patients to establish themselves with a designated Care Team. The Care Team will then work with you to develop a care plan to meet your health care goals. This model of care is state of the art and has been proven to improve quality of care while reducing healthcare cost through reduction in unnecessary emergency room and clinic visits.
